Canopy provides DAY and RESIDENTIAL services to people with intellectual and/or developmental disabilities (I/DD).

 

SHARED LIVING PROVIDER supports the individuals with developmental disabilities in the Shared Living Provider's home. Candidates must be able to provide person-centered supports to meet the needs of the people they serve throughout their lives and based upon the programming requirements outlined in their Individual Support Plans. Providers are also required to go through trainings that will teach them how best to support people in services in living fulfilling lives. 

First, candidates must apply and go through an interview.

 

Then, their coordinator will work with them to meet and possible support people looking for care that might be a good fit for one another.  Placement is not guaranteed and may take an extended amount of time.

 

Completing training will be an ongoing expectation.

How to Get Started

Duties Include:

  • Providing services in a manner that respects the dignity and rights of the individual, protects the rights of the individual, and ensures that the individual is free from abuse, neglect and exploitation

  • Ensuring that all services stated in the contract with Canopy and ISP are provided according to the needs of the individual

  • Ensuring all medical needs are being met in cooperation with residential entity, including scheduling, transporting, communicating with medical professionals, and ensuring all orders are being followed accurately and timely

  • Maintaining a clean, safe and suitable environment related to the needs of the person supported

  • Teaching and encouraging well-balanced meals, especially regarding modified diets

  • Providing safe and reliable transportation that meets the individual’s needs

Must be able to pass the following background checks:

  • Nebraska Department of Health and Human Services Adult Protective Services Central Registry

  • Nebraska Department of Health and Human Services Child Protective Services Central Registry

  • Essential Screening Backgrounds (private consulting firm)

Successful candidates must be:

  • Highly organized

  • Self-starters

  • Intermediate level computer skills 

  • Knowledge of “Therap” is highly recommended

  • Two years of experience in direct support field of developmental/intellectual disabilities is expected 

  • Applicants must be at least 21 years of age, possess valid identification, reliable transportation, and home/renter’s insurance

  • Candidates must have a working computer, printer, and scanner.
